When playing the C# I find it good to just move your 3rd finger up to 
the c# on the A string and keep the other fingers forming the D (or 
what's left of it).
       The letters in brackets are more the noted to be played, but I 
guess if you know the song, you'll know how it should be played, from 
these guidelines.
